La société actuelle tend à faire reculer d’année en année l’âge de la première grossesse. Au niveau biologique cependant il est une donnée qui ne varie pas : la fertilité décline avec l’âge. Maximale entre 25 et 29 ans, elle diminue de manière lente et progressive entre 35 et 38 ans, et plus rapidement après cette échéance. Ainsi à 30 ans, une femme souhaitant avoir un enfant a 75 % de chance de réussir au bout d’un an, 66 % à 35 ans et 44 % à 40 ans1. La fertilité masculine diminue elle aussi avec l’âge.

Programmer les rapports au moment de l’ovulation

Toute grossesse débute par la rencontre entre un ovocyte et un spermatozoïde. Or, cet ovocyte n’est fécondable que dans les 24 heures suivant l’ovulation. Pour maximiser les chances de grossesse il est donc important de détecter cette « période de fertilité ».

Sur des cycles réguliers, l’ovulation se situe au 14ème jour du cycle, mais il existe de grandes variations d’une femme à l’autre et d’un cycle à l’autre. Dans un objectif de conception, il est donc conseillé de détecter sa date d’ovulation avec l’une de ses techniques : courbe de température, observation de la glaire cervicale, tests d’ovulation.

Les spécialistes recommandent d’avoir des rapports au moins tous les deux jours autour de cette période y compris avant, car les spermatozoïdes peuvent rester fécondants dans les voies génitales féminines durant 3 à 5 jours. Ils auront ainsi le temps de remonter jusqu’aux trompes pour y rencontrer, éventuellement, l’ovocyte libéré lors de l’ovulation. Attention toutefois : ce bon timing ne garantit pas la survenue d’une grossesse. Sur chaque cycle, la probabilité de grossesse en ayant eu des rapports sexuels au moment clef n’est que de 15 à 20 % (2).

Eliminer les facteurs nuisibles à la fertilité

Dans notre mode de vie et environnement, de nombreux facteurs jouent sur la fertilité. Accumulés dans un « effet cocktail », ils peuvent réellement diminuer les chances de grossesse. Dans la mesure du possible, il est donc important d’éliminer ces différents facteurs, d’autant que la plupart sont néfastes pour le fœtus une fois la grossesse installée.

  • le tabac pourrait abaisser la fertilité féminine de plus de 10 à 40% par cycle (3). Chez l’homme, il altèrerait le nombre et la mobilité des spermatozoïdes.
  • l’alcool peut engendrer des cycles irréguliers et non ovulatoires et augmenter le risque de fausse-couche, tandis que chez l’homme, il altérerait la spermatogenèse.
  • le stress joue sur la libido et déclenche la sécrétion de différentes hormones pouvant avoir un impact sur la fertilité. Lors d’un stress important, l’hypophyse sécrète notamment de la prolactine, une hormone qui à des niveaux trop élevés risque de perturber l’ovulation chez la femme et chez l’homme, entrainer des troubles de la libido, une impuissance et une oligospermie (4). Des pratiques telles que la méditation (mindfulness) aident à lutter contre le stress.
  • la caféine en excès pourrait augmenter le risque de fausse-couche, mais les études demeurent discordantes sur le sujet. Par précaution, il semble toutefois raisonnable de limiter sa consommation de café à deux tasses par jour.

Bien d’autres facteurs environnementaux et habitudes de vie sont suspectés de jouer sur la fertilité : les pesticides, les métaux lourds, les ondes, le sport intensif, etc.

Avoir une alimentation équilibrée

L’alimentation a également son rôle à jouer dans la fertilité. De même, il est prouvé qu’un surpoids ou au contraire une grande maigreur peut altérer la fertilité.

Dans Le Grand livre de la fertilité, le Dr Laurence Lévy-Dutel, gynécologue et nutritionniste, conseille de faire attention à ses différents points pour préserver sa fertilité :

  • privilégier les aliments à faibles index glycémique (IG), car une hyper-insulinémie répétée nuirait à l’ovulation
  • réduire les protéines animales au profit des protéines végétales
  • augmenter l’apport en fibres alimentaires
  • veiller à son apport en fer
  • réduire les acides gras trans, potentiellement nuisibles à la fertilité
  • consommer des produits laitiers entiers une à deux fois par jour

Selon une récente étude américaine (5), la prise quotidienne d’un complément multivitaminé en période de conception pourrait diminuer le risque de fausse-couche de 55 %. Attention cependant à l’autoprescription : en excès, certaines vitamines peuvent se révéler néfastes. Il est donc conseillé de prendre avis auprès d’un professionnel.

Faire l’amour dans la bonne position

Aucune étude n’a pu mettre en évidence le bénéfice de telle ou telle position. De façon empirique, on conseille cependant de privilégier les positions où le centre de gravité joue en faveur du cheminement des spermatozoïdes vers l’ovocyte, comme la position du Missionnaire. De même, certains spécialistes recommandent de ne pas se lever de suite après le rapport sexuel, voire de se tenir le bassin surélevé par un coussin.

Avoir un orgasme

C’est également un sujet controversé et difficile à vérifier scientifiquement, mais il se pourrait que l’orgasme féminin ait une fonction biologique. Selon la théorie du « up suck » (aspiration), les contractions utérines déclenchées par l’orgasme entraineraient un phénomène d’aspiration du sperme par le col de l’utérus.
